From this screen you can view the disclosures from the school you have been assigned.  This is can be searched, sorted and exported to excel, and you can view, delete or add new disclosure forms.  To view a disclosure you will need to click the view link, to delete the disclosure you need to click the delete link next to the disclosure you want to delete.  To create a new disclosure you need to click the add disclosure button.


Disclosure



From this screen you need to enter the school, the date that the report is being created, the child's name, the child's date of birth, and any other information required for the disclosure.


Incident Data



Next you will need to enter the incident first suspected date, and the incident occurred date (these are the end of the required fields) next you will need to fill out all other information that may be required by your school district or by KSK administration.


Reporting Data



Next you need to enter the reporting data.


  • Oral Report to School Official
  • Oral Report to KSK Supervisor
  • Oral report to KSK Director of Operations and Instruction
  • CPS


Attachments


You have the ability to add up to 4 attachments to the system, to do this click the browse button under Attachments and select the file you wish to upload.



Reporter's Information


Next you can enter the reporter's information, instructor, school, city and the date.



Once completed you can then click the save disclosure button to save the disclosure.
